Behavioral Health Home 

The Behavioral Health Home (BHH) program is a community-based care coordination program whose primary focus is to improve the health and wellness of individuals who are identified as high risk due to their psychiatric illness. The program is designed to improve the quality of treatment, to improve the cost effectiveness of treatment, to reduce hospitalizations and trips to the emergency room and overall lead to improved health outcomes.The BHH is for individuals with diagnosed bipolar disorder or schizophrenia who live in Atlantic County and have a complicating medical condition like high BMI, high BP, or diabetes. The individuals must have active Medicaid funding or be Medicaid eligible.

SERVICES OFFERED

Behavioral Health Home services focus on a person-centered approach where each individual utilizes a multidisciplinary team to coordinate care and services while reaching his or her health and wellness goals. In accordance with guidance from the Centers for Medicare and Medicaid Services (CMS), the BHH at Crossroads consists of a team of professionals lead by a Registered Nurse.The team is available to provide care and consultation 24 hours a day for 365 days a year. The program provides a number of services that include:

Comprehensive Care Management

A collaborative process designed to manage medical, social, and mental health conditions more effectively.

Care Coordination

Implementation and monitoring of holistic plan to help identify appropriate linkages, referrals, coordination and follow-up to needed services and supports

Health Promotion Services

Encourage and support healthy living concepts to motivate individuals to adopt healthy behaviors and promote better health management and wellness

Comprehensive Transitional Care

Specialized care coordination services that focus on the movement of individuals between different levels of care settings

Referral to Community and Social Support Services

Identify and provide referrals including but not limited to, medical and behavioral health care, entitlements and benefits, respite, housing, transportation, legal services, educational and employment, as well as many other specialized and supportive services

Individual and Family Support Services

Activities, materials, or services aimed to help reduce barriers to achieving goals, increase health literacy and self-efficacy skills as well as improving health outcomes

BHH is not a residential program. It is a community-based case management program with the purpose of improving the health, wellness and resilience of individuals who are at the highest risk for psychiatric hospitalization such as those with Bipolar 1 Disorder or Schizophrenia or any other serious mental illness. The BHH treatment team provides services both on-site at the facility as well as off-site in the community.

BHH accepts self-referrals, referrals from within the agency as well as referrals from families and other community providers and entities. The intensity and length of service provided is different in each one of the three phases of the program including:

  • Engagement Phase
  • Active Phase
  • Maintenance Phase.

Eligible clients may receive program services for an indefinite length of time.

The agency provides a number of other services at this facility including Office Based Addiction Treatment (OBAT), Mental Health Partial Care Services, and Substance Abuse Intensive Outpatient Services. The BHH treatment team provides services both on-site at the facility as well as off-site in the community. BHH is located at:
